Description

Tobra-D® Eye and Ear Drops is manufactured by Laborate Pharmaceuticals. The active ingredients of Tobra-D® are Tobramycin, a water-soluble antibiotic from Aminoglycoside class, and Dexamethasone – a synthetic member of the glucocorticoid class of steroid hormones.

Indications:

- reducing eye inflammation (swelling, itching, pain, irritation) and preventing/treating bacterial infection following cataract surgery;

- bacterial ear infections (otitis externa followed by infected and inflamed ear, middle ear infection when the eardrum has become ruptured.

Tobramycin is active against Staphylococcus aureus, Citrobacter species, Enterobacter species, Escherichia col, Klebsiella species, Morganella morganii, Pseudomonas aeruginosa, Proteus mirabilis, Proteus vulgaris, Providencia species, Serratis species.

MOA (mechanism of action):

Tobramycin is a strong anti-infective agent that works by affecting the production of certain proteins by bacteria. These proteins are necessary for the bacteria's survival. Tobramycin causes the bacteria to produce abnormal and faulty proteins, which ultimately kills the bacteria.

Dexamethasone is a potent corticosteroid that effectively controls the inflammation by inhibiting the release inflammatory mediators. Namely, it involves phospholipase A2 inhibitory proteins, lipocortins, which control the biosynthesis of potent mediators of inflammation such as prostaglandins and leukotrienes.

Dosage

The recommended dose is 1 drop in the treated eye(s) every 4 to 6 hours while being awake.

During the initial 48 hours, your doctor may increase the dose to 1 drop every 2 hours.

Do not use for more than 24 days.

Precautions:

Tobra-D® Eye and Ear Drops are not for injection.

If you are using other eye drops or eye ointments, wait at least 5 minutes between using each one.

Eye ointments should be administered last.

Prolonged use of Tobra-D® Eye and Ear Drops may lead to glaucoma, cataract, and fungal infection.

Tobra-D® Eye and Ear Drops are associated with allergic reactions, even following a single dose. Discontinue using Tobra-D® immediately and contact your physician at the first sign of a rash or allergic reaction.

Consult the doctor if your symptoms get worse or suddenly return, you are diabetic, you are using other antibiotic treatment.

Intraocular pressure (IOP) should be regularly checked in children below 6 years of age receiving these drops in eyes.

After prolonged or intensive treatment with Dexamethasone containing drugs, suppression of the adrenal gland function may appear.

Avoid contaminating the applicator tip with eye, ear, fingers or another source.

Patients should not wear contact lenses while using Tobra-D® drops for eye infection.

Wash your hands before using the drops.

Missing a dose

If you miss a dose you should instill it as soon as you remember about your missing. If it is the time for the next dose you should continue your regular dosing schedule. Do not instill a double dose to make up for a missed one.

Overdose

If an overdose occurs and you are not feeling well, stop taking the medication immediately. You should seek emergency medical attention or contact your healthcare provider immediately.

Storage

Store at a cool temperature, away from excess moisture and heat. Do not freeze. Stop using the bottle 4 weeks after first opening. Keep out of the reach of children. Keep the bottle tightly closed.

  • itchy rash or hives
  • swelling of the face, lips, tongue or other parts of the body
  • shortness of breath/wheezing
  • severe skin reactions such as blistering which may be accompanied by sore throat

Contraindications

Do not use Tobra-D® Eye and Ear Drops if you are allergic to Tobramycin, Dexamethasone, other Aminoglycosides, other Corticosteroids or to any other ingredient of the medication.

Discontinue using Tobra-D® Eye and Ear Drops immediately and contact your physician at the first sign of a rash or allergic reaction.

Do not use Tobra-D® Eye and Ear Drops if you have a history of severe allergic reactions associated with using antibiotics.

Do not use Tobra-D® Eye and Ear Drops if you have a disorder causing a thinning of the eye tissues, such as rheumatoid arthritis, Fuch’s dystrophy or following a corneal transplant.

Do not use Tobra-D® Eye and Ear Drops in combination with non-steroidal anti-inflammatory drugs (NSAIDs), ritonavir.

Do not drive or operate machinery until your vision is clear while using Tobra-D® Eye and Ear Drops.

Do not use Tobra-D® Eye and Ear Drops if you have or think that you have any type of infection of the eye, a sticky discharge from your eye, a red eye that has not been seen by a doctor.

Do not use Tobra-D® Eye and Ear Drops if you have IOP (intraocular pressure), fungal or viral infections of the eye, tuberculosis infection affecting the eye, eye conditions producing pus.

Do not to use Tobra-D® Eye and Ear Drops if you are pregnant, plan to have a baby or are breastfeeding.
